            MOST PATIENTS WITH THORACIC AORTIC aneurysms are asymptomatic. Some, however, may become symptomatic due to compression of the adjacent structures. Airway compression causing positional dyspnea is rare and poses significant risks and challenges in airway management.1 Furthermore, prolonged periods of circulatory arrest often needed to perform aortic arch repair, raises concern for cerebral protection in such settings. We describe a rare case of positional dyspnea secondary to airway compression by a large aortic arch aneurysm that required urgent airway intervention.
