The Anterior Mediastinal Mass—the Challenging Management of Hypoxemia Despite Peripheral Cannulation for ECMO

Published:March 05, 2021DOI:https://doi.org/10.1053/j.jvca.2021.03.003
ANTERIOR MEDIASTINAL masses of the mediastinum are well-recognized causes of perioperative airway obstruction that may be associated with mortality and significant morbidity.

Depending on size and location, a mediastinal mass also may result in compression of the great vessels.

General anesthesia is particularly high risk in this clinical setting and may precipitate life-threatening hypoxemia and/or cardiopulmonary arrest.

The anesthetic management of the patient with an anterior mediastinal mass.
Several management strategies have been proposed to navigate this challenging scenario, including anesthetic induction in the sitting position, maintenance of spontaneous respiration, and availability of venoarterial extracorporeal membrane oxygenation (VA ECMO).
